This unit has been installed in a 2001 Tundra for 9 years and has worked flawlessly. The two way remote went bad a couple of years ago so I use a one-way fob. Have never had any issues with the unit or the fob. Howerver, yesterday the truck started on it's own as I was unloading something from the bed. I thought I accidentally pressed the start button on the fob in my pocket and just shut it down with the fob. Then at 4:30 in the morning I heard my truck start in the driveway! After making sure nobody was stealing it, I shut it down with the remote. Later, around 10am, my wife called me at work and asked why my truck was running in the driveway. It self started again. I had her shut it down with the remote. When I got home, I activated the hood-open switch to disable the remote start function. I don't even know where the unit is installed in the truck. I assume under the left side of the dash.

Has anyone had this issue and is there a known cause? Could a faulty key fob be the culprit?

Its a possibility that you could have accidentally put it in timer start mode with the remote. When that happens the truck will start every 3 hours. You can disable it by starting the truck with the key and stepping on the brake. I had the same thing happen to me with my 771. Hopefully this is what the your issue is.
