My new head unit has a hi volt pre out of 16 volt balanced.  What does a hi volt output actually do???

They kinda explained it at the shop but I really diden't understand

Sounds like you probably have an Eclipse?  You can read all about pre out voltage here https://www.eatel.net/~amptech/elecdisc On the right hand side scroll down and click on Pre-amp Output Voltage

16 volts???? What head unit is this??

That’s an excellent head unit and I love the sound quality they put out . I just wish that they were a little more up on the times. It would be nice to see the Eclipse's  have an Mp3 player or satellite radio. But it doesn’t look like that’s going to be happening this year.

I'd have to say that for most amps it seems that a transformer or a voltage divider or a simple opamp circuit would be required. I haven't seen too many that would like such a high voltage...
