Two of the wires of the LOC are connected to the wrong speaker wires, and you are getting a cancelled signal when you plug both RCA's in.  There is a wire that should be connected to the positive that, instead, is connected to the negative speaker wire.  And the neg is tapped into the pos.  Reverse these two connections.

That was quick!  Glad to be able to help.  Just to check if sub's in phase with rest of the system, if the amp has a 180 switch, try it both ways.  If it doesn't, reverse the speaker leads from the sub to the amp to switch phase.  The proper phase is when the sub is loudest and deepest with the rest of the music.  This is just to make sure you reversed the proper wires.

I can't figure that one.  I'll bump this up so someone will take a look at it.

Plug bass remote into the amp, turn to max., then reset the gain on the amp.  But first turn off the bass boost.  I think the bass remote is whacking out the gain setting, and the amp is running hotter at a clipping level.  So the fan is whining.  You should already have set the gain once, so turn everything back to the starting line and do it again with that remote in.  See if that does the job.

Good suggestion from stevdart, highly recomend you do this.  But it could also simply be the fan is just noisy, or it has a bearing that is going out.
