How do I access the rear speakers in a 2003 Corolla? 

It looks like you have to tear a lot apart but once that is done it looks like just an ordinary top mount.

Yeah you do. Also look under the rear deck and you might see some "christmas tree" type door panel retainer clips holding the rear deck down. Pop those up through the hole and you should be able to lift out the rear deck.

How about the front doors?  Wheres the screw to get the panel off?

On the front panel have clips that attach the panel to the door. You just have to go alont it and slowly pull them off. Take your time.
