The factory nissan ground wire is useless.  Run a ground wire directly to the chassis and you should have yourself a working cd player.

Here is the correct wiring diagram for your truck, make the proper connections, and you shouldn't have any problems.

Be sure to check your wires with a DMM before making connections. If you still have power failure after that, then check all of your fuses.

some of the wires in the chart for the 89 nissan don't match mine also maybe it's becuse i have a 4x4 would that make a differance ? also do i have to hook all the speaker wires for it to power up ? thks

All you need to power it up are constant 12v, switched 12v, and ground.

To test to be shure the cd player is working twist the 2 12v wires together on the cd player (red and yellow)  touch them to the positive post on your battery, and the black (ground) wire to the ground post on your battery.

I still say your haveing a ground problem.  How did you test it?  did you just touch the ground wire? or did you actually put on a ring terminal and drill a self tapping screw into the metal frame back there?
