I have a 520t backup battery installed. My question is if the main battery is disconnected, does only the brain receive the power or do all the additional sensors/sirens also get power? I am assuming if the additional sirens are powered with a relay, they will not sound if the main battery is disconnected?

If powered via a relay the sirens will sound once the relay is taken of the feed from the alarm, if you were to ground the relay on the body of the vehicle chances are it won't trigger the sirens, the battery isn't that powerful, also adding more than one siren will decrease battery lifespan on that 520t significantly.
