My customer has stated he is getting false siginals when activating start and lock functions.

As in he activates start and gets response that shows car is running when it is not.

Same when he locks vehicle, phone confirms lock and actually is not.

We rebooted antenna receiver a shile back for intermitant issues and it cleared up other issues then.

Do I repeat that process? Or is there other things going on??

What system is the Carlink interfaced with?
Is it interfaced via data or hardwired?

System is on a 2010 Nissan Armada, Excalibur RS-350-EDP+, OL-MDB-ALL (MDBI-NI4-EN), Omega Carlink, all data. original install 10/2012. Has been working fine until 2 weeks ago.

I have only dealt with avox and directed units myself but some basics should apply to any unit using cell service. Has the location of the vehicles primary use changed at all? Moving to a new home/job? Taken any long trips? It is possible that changing towers has caused a new issue. Remember these are typically pretty limited cell devices. Is the network being upgraded in that area? If so devices like car link will usually be lower priority than monthly subscribers. It is important to consider all factors here. If the device works in the shop then there is usually no problem with installation. Consider where the unit is located in the dash as well. It is best to treat it as a gps antenna getting it up as close to the top of the dash as possible and center of the windshield away from the A pillars. Obviously it is not using satellite or direct line of sight, but providing a better path can never hurt. Hope it helps.

Re-reading that OP it is possible there is a failure in the remote start telling the cell unit it has started when it has not. Are you running tachometer? Is there a pulse to start wire like on viper units? ground activation on the remote start. When it fails are the parking lights on but the engine is off, or has the vehicle not responded at all?

Reset everything on the system while leaving the car link unhooked. After the remote start is working the plug in the car link and reset that as well. Let me know if that works.

The Audiovox and Omega/Excalibur Carlink units are the same.
The responses received in the app only confirms that the Carlink in the vehicle has received a command and does not confirm that the command has been carried out successfully.
First confirm that the 350+ works with the remotes. Than have the customer log in to the Carlink site and test the functionality from the web. If it works from the web, uninstall the app and re-install to see if it corrects the problem. If re-installing the app does not solve the problem, check to ensure the connections are secure for the installation. Make sure the data port of the 350+, green connector of the Carlink and pins are not damaged. Try swapping the ports for the Carlink and OL module.
An additional test that can be performed is to log into you Carlink dealer account and switch the installation to analog setup and assign an auxiliary function to one of the analog wires. Once that is done, test that you it goes to ground when the activated within the app. If it works in analog mode, there may be a problem with the serial/data communications of the unit.
One last thing you can do is to check that the firmware of the RS-350+ is up to date. You can connect the black port of the 350+ to the Omegalink/iDatalink updater and log into your OL account to see if there is an update that can be flashed.
