I am having an issue with a PKALL on a 2012 Lincoln Navigator in w2w configuration

I have verified correct connection of TX/RX wires
+12v and GND wires are connected correctly
Pink ignition wire is connected
GWR wire is connected to remote start

I am able to go through the programming procedure. Hold the button on the PKALL, plug in both connectors, insert 1st key for 3 seconds, insert 2nd key for 3 seconds, then remote start. The vehicle will start and the PKALL will flash the LED 10 times verifying it has been programmed. I shut down the vehicle, try to remote start again and get a flashing theft light and a no start. Vehicle displays "Key not programmed" on display.

I have tried erasing all of the keys stored in the vehicle using Ford IDS. Same situation, module seems to program then doesn't work a 2nd time
