They are T-Taps, same technology as a Scotchlock but instead of inserting both wires into the device, you crimp the T-tap to the source wire and then you insert a fully encapsulated male spade into the end of the T-tap. I noticed that there are no fully encapsulated male terminals. Could this be the problem? I am with Kevin, my crystal ball is down at the moment as well.

Whats wrong in the pics is the t-taps lol. This was back when I didnt know any better LOL!
