I was wondering if there is a way to wire a relay to turn on after seeing a 12v D.C. Switched source? Basically I need the relay to turn on about 20 sec after seeing the 12v switch source.

Or use a DEI 528t

Did the research the 528t turns on when it sees the switch voltage. I need a relay that will turn on after 15sec after seeing the switch voltage and only stay turned on for 5 sec.

In your original post you said nothing about only staying on for 5 seconds. The thing I posted will do what is specified in your original post. It will remain on as long as 12 volts is applied.

Use 2 X 528t in series and next time remember to tell us about staying on for 5 seconds, it might have helped.
