Unfortunately, you have been provided a set of terrible instructions. The fact that there is a test light in the diagram shows how dated this is.
I digress.....

This is a standard 5 wire setup.
When the "green" relay is active, 12V will be applied to the motor and the assembly will move "anti-clockwise"
When the "yellow" relay is active, 12V will be applied to the motor and the assembly will move "clockwise"

It has me confused the motor has 4 wires green,yellow,red and black. Car has a constant and two switch wires one gets hot on up and one gets hot on down.

Original Instructions

Test it this way first
Apply 12V to the motors red lead
Apply Ground to motors black lead
When 12V is applied to the motors green lead and ground is applied to the motors yellow lead the motor will move counter-clockwise
When 12V is applied to the motors yellow lead and ground is applied to the motors green lead the motor will move clockwise

You should be using a 5 wire relay setup for the green and yellow wire leads

Note: I cannot say if the motor will get wizzed if you apply 12V to the red wire at all times.....
