I know this is an old topic but thought I could be of some help to someone else who might need this info.  There is a small hole on the round base where the visor arm attaches.  I use a 90 degree pick tool (Snap-On) that has a very strong tip and place it in that hole on the base.  You have to turn the visor left to right until it gets lined up properly.  Basically, you push in with the pick and turn the visor several times and work it out.  You have to be carefull though, because it is just a flimsy plastic assembly and it could be damaged easiliy.
