I want to add an armrest, up near the top of the door where you rest your arm for the low rider look.  Right now there is just smooth door panel there, not molded plastic, because I have a 1965 Ford. Can anyone give me some ideas how to build and mount an elbow rest?

We may need a little better explanation of the "Low rider look" to be able to help you.

Well, maybe not really low rider, but something that you can comfortably lay your left elbow on when you're driving with the left hand.  I was thinking of mocking up a structure with paper mache, making a mold with fiberglass on that and then laying the mold back on the door and filling it with kitty hair or maybe expanding foam.  Then there is the problem of fitting a cushion on top of it in a nice looking way and also fastening it to the door.  Well, that's why I'm here , to get ideas on how to build such a thing.

Can you provide a picture of your door/interior as-is?
I have not failed. I've just found 10,000 ways that won't work.

you could check suppliers of classic car interiors not necessarily only fords i know vw bugs have a pretty small armrest/handle to close the door if you dont like the look just use the top cushion or check salvage yards
