when you hook one of these up to the factory radio do you typically ground it. If its not grounded what could happen.

A passive line-out converter (such as you hook up to your factory speaker wires) is typically NOT grounded, nor should it be if it does not have a "ground" connector.  A powered line-out converter with a line driver IS grounded as part of the power supply system.

Does anyone know if line drivers are good or not? They are suppose to up the voltage in the rca line coming out of the deck to the amp right?

They can be very good depending on the driver and the amp.  Some line drivers put out such a high voltage that some amps cannot handle it.  The idea is to allow a cleaner signal to get to the amp at a high enough voltage that the amp input gain settings can be kept very low.
