I haven't done Odyssey sliding doors yet, although I will, becuase my mother is buying one of those vans soon.
But anyway, to answer your safety concern:
Honda keyless doesn't work with the ignition on, as you already know.
Most aftermarket alarms and remote starters will operate while the car is running under remote start, so you can enter/exit the car....... but when the car is running under its own power, the aftermarket remotes are inoperative.
If the user presses the remote while the car is running under its own power, nothing will happen.

By looking at the electrical diagram of the Honda Odyssey, you can't operate the doors if the vehicle is not in "PARK" or the vehicvle is moving.
Both Sliding doors control modules (Rear Quarter, each side) have to receive a ground signal by placing the shift lever in Park. They also receive signals from the Vehicle Speed Sensor.
For diagram 1085, the door switch are in located in the rear opening of each sliding doors and can only be accessed by the door jamb opening or by removing the rear quarter trim on each side to get to the wire that connects to the modules.

The first one I got from this site, and I see how the set up works for the most partm but I will be using 2 aux. outputs so I will have to tweak that setup a little. Anyway, The first thing I want to know is if I hook up my auxiliary outputs according to the diagram, will this bypass the ignition feed to the keyless entry module and allow the doors to open even with the ignition on. I am fine if the factory transmitters will not, but I am curious if the aftermarket will. and if they will, I guess I better make damn sure that those auxiliary channels dont get bumped while driving down the freeway. However unlikely that is, It is something to think about, I mean I think the aux outputs require the buttons to be held down for a few seconds, but it is not entirely unreasonable to accidently do that if you squish the second transmitter in your pocket.

i did a couple of this setup and is not that hard,and you just need 2 aux output's to do the job,on both door's.one for each door,to open and close.DEI send me a diagram,but is not rigth,so a make my own setup,and it work's fine.
