avital 4103 not programming

Installing Avital 4103 and DBALL2 on 2004 Chevy Express 3500. There are a couple of problems - FOBs will not program and D2D will not program.
I connected per instructions. Had the DBALL2 flashed. I can connect the Activation Input to ground and the DBALL2 activates and the 4103 relays click. The accessories comes on, but it won't crank. If I put the key into run and connect the Activation Input to ground, the van will start.
I tried to program the 4103 for D2D. When I disconnect power, ground the Activation Input (WHITE/ Blue), and connect power, I get no LED light. However, when the unit is connected, I can use the valet button and get LED indication. Which leads to the other problem. Even though I get LED indication for FOB programming, the FOBs will not operate. I used the sequence for clearing the remotes and tried reprogramming but it did not work either. I also tried resetting to all factory defaults but still no lights when trying to get to D2D mode.
Any ideas what is going on? Is the 4103 bad?

Is the antenna plugged in.
